Complete this form to review your current support adjustments with the Disability Advice and Guidance Team.
Do you have a Student Support Information Sheet (SSIS)?
All students who have registered with the Disability Team will have a Student Support Information Sheet (SSIS). An SSIS is the document that details your recommended reasonable adjustments.
If no:
- Register with the Disability Team by uploading your medical evidence/diagnostic assessment documentation via the Student Services HUB. Then a member of the Disability Advice and Guidance Team will be in contact with you to complete your SSIS.
- Or speak with a Disability Coach via live chat, or from 11am-3pm Monday - Friday (during term time) in the Alsop building.
If yes:
Answer the following questions to request a review with the Disability Advice and Guidance Team:
NOTE: If you have an SSIS and are looking to add an additional diagnosis, you must upload the supporting evidence for that diagnosis to The Student HUB. This will prompt a member of the Disability team to reach out to you.
